Chronic Disease is a long-lasting condition that can be controlled but not cured. As described by the CDC, chronic disease is the leading cause of death and disability in the United States. It accounts for 70% of all deaths in the U.S., which is 1.7 million each year. Although chronic diseases are among the most common and costly health problems, they are also among the most preventable and most can be effectively controlled.

We pride ourselves in providing strong continuity of care for all our patients. We will make appropriate referrals to assist in the ancillary care that is needed in some disease states. We ask our patients to schedule medication reviews every 3 to 6 months with our providers.
During your evaluation, our providers will determine if a consultation with a specialist is warranted. We have an excellent working relationship with the sub-specialists in the area and will ensure that your healthcare services are seamless.
We offer all our patients online access to their results after they are reviewed by the provider. Via the portal, the patient has direct communication with our providers, request medication refills and schedule their next appointment.
